RAID5 Maxtor 500GB MaxLine Pro 500 SATA 3.0 -- HD keeps "failing"

Folks,
I'm running 3 500GB Maxtor MaxLine Pro HDs in a RAID5 configuration.

One of my drives keeps failing and the RAID keeps rebuilding itself.  I've tried to eliminate the RAID card by swapping in another of the same model (what are the chances that I have 2 that are bad?)  I've also tried to plug the one particular drive into a different channel and I still get the same HD having to be rebuilt.

So I took out the "bad" HD, and tried to run the SeaTools on it.  I tired the DOS version, but it doesn't detect the SATA HD.  So I ran the HD version.  It detects the HD, but any tests I run, it fails.  Is the HD really that bad?  I used the same tool on a WesternDigital SATA and it detects it, and runs thru the tests just fine.

My questions are:
1.  Is my Maxtor HD bad?
2.  How do I run a utility to test this?
3.  Is it because the format isn't "recognizable," that SeaTools doesn't run properly on it?  (or is it because the MBR is weird due to it being setup for RAID?)

Any help would be appreciated!

Thanks!
